What tests are required for the "pre-cycle lab work" needed to qualify for the IVF Refund Plan?

In order for PFC to share some of the financial risk under its outcome based plans, there have to be three factors present: the ability of the female partner to produce eggs, the ability of the male partner to produce sperm, and a healthy uterus into which the embryo can be implanted. The availability of eggs and sperm are confirmed by a variety of blood tests performed on the female and male partners and by a semen analysis. The suitability of the uterus is usually determined by performance of diagnostic hysteroscopy or ultrasonography. It is not necessary that these tests be performed by PFC.  In fact, many of our patients choose to go to their own physician or laboratory and have the results forwarded to the PFC clinical staff. During your clinical and financial coordinator consultations, you will receive more detailed information on required testing.

In the event the pre-cycle lab work shows one or more of the necessary factors to be missing, the use of an ovum donor or sperm donor may still allow participation in the IVF Refund Plan.

How is it determined that IVF is medically indicated?

The initial consultation with a PFC physician will include a review of your medical history and a discussion of treatment options. The PFC physician, to help determine if IVF is medically indicated for you, will also review medical records of your prior treatments.

How much of my IVF cycle will insurance cover?

Insurance coverage for infertility treatments such as IVF varies widely between insurance plans. While coverage for IVF is not that common, you should check with your carrier or your employees' benefits manager. If you do have some level of coverage, our staff will work with you to obtain a predetermination and/or preauthorization from your insurance company before you begin your cycle of treatment.

I am 47 years old but my ovum donor is 34. Whose age determines the applicable category of the IVF Refund Plan?

The age of the egg provider determines which category will apply.
