For some patients, who either do not qualify for or do not want a package plan, PFC offers IVF Services on a fee-for-service basis. This plan is applicable to those using their own eggs and uterus, ovum donation and those women utilizing a gestational carrier. Insurance coverage may be applied to this plan.

In-cycle services provided under plan:

• medical services provided by a PFC physician in our facilities, including clinical monitoring, mock embryo transfer (MET), egg retrieval, anesthesia during the egg retrieval process, embryo transfer and in-cycle medical consultations related to IVF
• standard services of the IVF laboratory, including the in-vitro fertilization procedure

Services not covered by plan:

• initial physician consultation
• pre-cycle evaluation including lab testing and mid cycle mock embryo transfer (MET)
• medications for ovarian stimulation and preparation of the uterus for optimal implantation
• cryopreservation and storage of embryos
• frozen embryo transfers
• beta hCG (pregnancy) blood tests and early obstetric ultrasounds

Special services not covered (not required by all patients):

• on-site services of a non-PFC physician, such as a urologist performing a Testicular Sperm Extraction (TESE) procedure.
• other monitoring or medical treatment provided by a non-PFC physician and / or facility
• assisted fertilization laboratory services such as Intracytoplasmic Sperm Injection (ICSI) or other specialized laboratory services.
• fees for donor sperm, ovum donor or gestational carrier
• third-party parenting agency fees

All fees are subject to change without advance notice. If the couple or individual and PFC have executed a financial agreement, the price stated in the agreement will be honored for the cycle, regardless of any change in fees occurring after the document was signed. Prices effective 12/01/12.
